If your members would like to attend, order a box lunch or brown bag, please have them print their names on the form below and indicate their preference. Have one local member of your fraternity collect the $10 and submit the total of all your fraternity members on the day of the event.
The completed form itself should be emailed to me no later than Monday, Sept. 18.
